1. I am the owner or agent of the pet named above. I hereby request and authrozie the Jurnee's Journey Foundation and the vet hospital or clinic of their or my choosing to perform an operation of sexual sterilization of the animal named above through whoever veterinatian(s) they may delegate.
2. I understand that the operation I have elected presents some hazards, and the injury to, or death of, an animal may conceivably result, for there is some risk in the procedure, and some risk is the use of anesthetics and drugs provided for the procedure.
3. I either certify that my animal has been vaccinated within one year prior to this date, or waive my right to proteect my animal by having it vaccinated, or request recommended vaccinations at the time of surgery. I understand that it takes up to two weeks for vaccinatios to protect my animal.
4. I understand the inherent risks of failing to maintain current vaccinations and waive all claims arising out of , or connected with, the performance of this operation due to such failure.
5. While every precaution will be utilized, I understand in the event that if my pet develops kennel cough after surgery, I am responsible for treatment at my own cost.

6. I understand that any vet, hospital or clinic has the right to refuse service to any animal to whom the surgery is deemed a health risk.

I also understand that my animal will not receive pre-operative bloodwork, and waive my right to have this service performed prior to surgery at a full-serivce veterinatiran.
8. I understand that some factors significantly increas surgical risk, including, but not limited to pregnancy, heat, and diseases such as felince immunodeficiency virus (FIV), feline leukemia (FeLV), and hearthworms.
9. I undesrtand that if my animal is pregnant, the pregnancy withh e terminated at surgery.
10. I understand that if my animal has an ipen umbilical hernia, it will be repaired at the time of surgery at an additional charge of $10.
11. I understand that if I do not retreive my pet at the agreed-upon time, the JJF, vet and/or clinic will exercise the right to turn the animal over to the nearest animal shelter or rescue. Owners of pets left after the agreed date shall be charged a boarding fee of no less than $10 per night.
12. I understand that my animal may receive a small permanent tattoo on his/her underside to show that he/she has been sterilized.
13. I hereby release the JJF, all veterinarians, technicians, assistants, directors and employees from any and all claims arising out of, or connected with, the performance of this procedure or any adverse reactions from vaccinations. I agree that I have not and will not claim any right o compensation from them, or any of them, or file action by reason of such sterilization or attempted sterilization of such animal or any consequences related thereto. I hereby agrees to indemnify and hold the JJF and any above listed party harmless for any damages caused during the transportation of the animal, or for any damages caused by any unforeseeable events including, but not limited to, fire, vandalism, burglary, extreme weather, natural disasters, or acts of God.
